Weight Loss Injections

Complete Guide to GLP-1 Injections, Results, and Costs

What Are Weight Loss Injections and How Do They Work?

Weight loss injections are prescription medications administered under the skin that help you lose weight by suppressing hunger and improving metabolic health. The most advanced and effective weight loss injections are GLP-1 receptor agonists—medications that mimic GLP-1 (glucagon-like peptide-1), a hormone naturally produced by your body. This hormone plays a crucial role in hunger regulation and metabolism.

GLP-1 is one of the most important hormones for weight management. It is released by the small intestine (by specialized L-cells) when you eat food, particularly food containing proteins and fats. This hormone signals the satiety center in your brainstem (the hypothalamus) to tell you that you are full, that you have eaten enough, and that you should stop eating. GLP-1 receptor agonists significantly enhance this natural signal, resulting in greatly reduced hunger, automatic lower food intake, and improved control over eating behavior.

The pharmaceutical history of GLP-1 medications began in 2005 with approval for type 2 diabetes, because they are very effective at regulating blood sugar. However, research between 2016 and 2021 revealed that GLP-1 medications work far more powerfully for weight loss than initially expected. This led to the development of specialized formulations with higher doses specifically designed for weight management. These special versions (such as Wegovy, Zepbound) have double to triple dosing compared to diabetes versions, delivering much better weight loss results.

The GLP-1 market has grown explosively from a few thousand users in 2021 to more than 5 million people worldwide by 2023. This rapid growth reflects the success of these medications: users lose an average of 15-22% of body weight in one year, which is significantly better than all traditional methods (diet: 5-10%, exercise: 3-7%, behavioral therapy: 8-12%). This makes GLP-1 injections currently the most effective non-surgical approach to weight loss.

Biological Mechanism

GLP-1 receptor agonists bind to GLP-1 receptors in many places throughout your body: the hunger center in your brain (hypothalamus), the stomach, pancreas, and intestinal tract. This multi-organ effect provides synergistic benefits that are greater than the sum of their parts. GLP-1 binding in the hypothalamus reduces hunger, binding in the stomach slows digestion, binding in the pancreas improves insulin production, and binding in the intestines improves nutrient absorption.

What Weight Loss Injections Are Available?

1. Semaglutide (Ozempic, Wegovy, Rybelsus)

Semaglutide is currently the most widely used and most studied GLP-1 receptor agonist in the world. It was the first medication in this class and has the most clinical evidence. Ozempic is the diabetes version (0.5-1.0 mg per week) and Wegovy is the specialized weight loss version (2.4 mg per week). Both are chemically identical, but Wegovy is dosed higher and is specifically designed for weight loss rather than blood sugar management.

Semaglutide is injected once weekly subcutaneously (under your skin), usually in the abdomen, thigh, or upper arm. Its mechanism works based on a long half-life of approximately 7 days, which means one injection per week keeps your body tissues saturated with the medication and constantly active. The injection itself is easy: a pre-filled pen with a very fine needle (33 gauge), much smaller than insulin needles.

Effectiveness is excellent: users achieve an average of 15-18% weight loss in 68 weeks (approximately 15-16 months) according to large clinical trials. The dosing schedule starts low (0.25 mg per week) and gradually increases to the full therapeutic dose of 1.0 mg per week (Ozempic) or 2.4 mg per week (Wegovy). This slow buildup protocol over approximately 4-5 months helps minimize side effects and allows your body to adapt.

Advantages of semaglutide: extensive clinical evidence from thousands of patients, more than 7 years of practical experience, very reliable action, one injection per week, well-studied long-term safety. Disadvantages: cost in Netherlands EUR 400-500 per month (uncovered by insurance), slightly higher underdosing risk compared to tirzepatide. This makes semaglutide a solid choice for many patients.

2. Tirzepatide (Mounjaro, Zepbound)

Tirzepatide is the newest and most innovative medication in this class, approved by EMA in 2022 and by FDA in 2023 for weight loss. The major advantage of tirzepatide is that it works on TWO hormone receptors: both GLP-1 and GIP (glucose-dependent insulinotropic polypeptide). This dual-receptor mechanism is why it works better than semaglutide for many patients.

Like semaglutide, tirzepatide is injected once weekly subcutaneously. It has a slightly shorter half-life (approximately 5 days) but accumulates in your body, so weekly dosing is optimal. The medication becomes more potent as you increase your dose, and patients can use a 12-week titration protocol (much faster than semaglutide) to reach full dose quicker.

Effectiveness is remarkable: users achieve an average of 20-22% weight loss in 72 weeks. In direct comparisons (head-to-head trials), tirzepatide was statistically better than semaglutide. The dosing schedule starts at 2.5 mg per week and increases to a maximum of 15 mg per week. At 15 mg dose, some users even achieve 25%+ weight loss.

Advantages of tirzepatide: more effective than semaglutide, faster titration protocol (12 instead of 20 weeks), dual-receptor mechanism provides better metabolic control. Disadvantages: newer medication (fewer long-term data), cost EUR 500-600 per month, higher dosing potential could cause more side effects in sensitive patients. This is the preferred medication for patients wanting maximum weight loss.

3. Saxenda (Liraglutide)

Saxenda is liraglutide, an older medication approved in 2014 for weight loss. It was actually the first GLP-1 medication specifically for weight management. However, it is significantly less potent and practical than newer alternatives. Liraglutide is a shorter-acting GLP-1 agonist with a half-life of only approximately 13 hours.

This short half-life means you must inject DAILY instead of once per week. This makes it much less practical than semaglutide or tirzepatide. You need 365 injections per year instead of 52. Dosing ranges from 0.6 mg to a maximum of 3.0 mg daily. This daily dosing requires more discipline and precision.

Effectiveness is lower: users achieve an average of only 8-12% weight loss in 56 weeks. This is considerably less than semaglutide (15-18%) and much less than tirzepatide (20-22%). This makes it less attractive for patients wanting maximum weight loss.

Advantages of liraglutide: longest clinical experience (more than 10 years), very well-studied safety profile, slow titration protocol helps minimize side effects. Disadvantages: daily injections very impractical, significantly lower effectiveness, cost EUR 300-400 per month, far fewer users now due to better alternatives. This is not the preferred choice for most new patients.

For most patients starting weight loss injections now, doctors choose between semaglutide and tirzepatide. The choice depends on your health, goals, and preference. Semaglutide is more conservative and good for those more sensitive to side effects. Tirzepatide is better for those wanting maximum weight loss and who can handle potentially slightly more side effects. Liraglutide is rarely chosen now due to daily injections and lower effectiveness.

How Much Weight Will You Lose?

The effectiveness of GLP-1 injections is extensively scientifically documented. In large randomized controlled trials (2021-2023), semaglutide users achieved an average of 15-18% weight loss in 68 weeks (approximately 15-16 months). For tirzepatide, results were even better: average 20-22% weight loss in 72 weeks. For context: traditional approaches achieve much less—diet alone: 5-10%, regular exercise alone: 3-7%, behavioral therapy: 8-12%, gastric bypass surgery: 30-35%.

Effectiveness is not uniform—it depends on multiple factors. Your starting weight plays a major role: patients with higher starting weight often see greater percentage weight loss because their absolute weight loss has more impact. Metabolic health is important: patients with insulin resistance often see more weight loss. Genetics play a role: approximately 90-95% of users see significant weight loss (more than 5%), but approximately 5-10% see minimal effect. In that case, we usually adjust medication (from semaglutide to tirzepatide) or dosing.

Weight loss does not follow a linear curve. In the first 2-3 months, many users see faster weight loss: 2-3 kg per month. This is because your body adapts to the medication and hunger is strongly suppressed. After months 3-4, weight loss gradually slows to approximately 0.5-1.5 kg per week. Around months 4-6, many people see a plateau: weight stabilizes. This is NORMAL and not a sign of failure. Your body is moving toward a new equilibrium.

Long-term results are very encouraging. Studies of patients using GLP-1 for 2-3 years show they maintain their weight loss as long as they use medication. However, if you stop: you gradually start regaining weight. On average, patients who stop regain approximately 2-4 kg per year. But many maintain 50-70% of their weight loss because the lifestyle changes (healthy eating, exercise) they have learned and that have become part of their routine remain.

How Does the Injection Process Work?

GLP-1 injections are specially designed for self-administration by patients at home—you do not need to visit the clinic for each dose. The injections use very fine needles (31-33 gauge), much finer than insulin needles (26 gauge) that diabetes patients use. These fine needles mean most patients barely feel anything. In our clinic experience, more than 90% of patients say: 'I barely felt it' or 'I felt a small prick.'

The procedure is simple and takes less than a minute. The needle is already attached to the pen—you don't have to set anything up. You choose an injection site (abdomen, thigh, or upper arm), wash the area with an alcohol wipe, and inject at approximately a 45-degree angle. Once the needle is in your subcutaneous tissue (the layer under your skin), you activate the injection mechanism (usually by pressing a button), which injects the medication over approximately 5-10 seconds. Then you remove the pen and dispose of it in a needle container.

Training is essential and Go Leaner provides this at your first appointment. A trained nurse or doctor shows you how it works, answers all questions, and lets you practice with a dummy pen (no needle). After one or two practice sessions, most patients feel confident. If you are afraid of needles or blood, we provide psychological support and can help you gradually reduce your fear.

Safety is built in. Needles are always single-use—you never reuse them. Special needle containers with safety mechanisms for injection needles are provided, so there is no risk of needlestick injury or infection. You keep the needle container at home until it is full, then Go Leaner provides instructions on how to safely dispose of it through your pharmacy or waste collection.

Are Weight Loss Injections Safe?

GLP-1 injections (semaglutide and tirzepatide) are safe when used under medical supervision. Semaglutide has been in clinical use since 2017 and has safely treated millions of patients. Tirzepatide since 2022. The safety profile is good: serious side effects occur in less than 2% of users. This is much safer than many other medications and certainly safer than weight loss surgery.

Common side effects occur in 40-70% of users. These are typically MILD and temporary: nausea (especially in first 1-2 weeks during dose buildup), diarrhea or constipation, abdominal pain or discomfort, fatigue. These virtually always disappear after several weeks as your body adapts. Nausea usually disappears after weeks 3-4. Diarrhea or constipation usually disappears after weeks 2-3. The key is slow dose buildup.

How to minimize side effects: Go Leaner uses a slow titration protocol (4-5 months) that starts with low dosing and gradually increases. This is much better than rapid dosing. Eat small meals instead of large portions. Drink plenty of water for hydration. Avoid fatty, greasy food initially—fat can worsen nausea. Feel free to eat plenty of fruit and vegetables—many patients feel natural aversion to fatty foods. Go Leaner's nutrition coaches help with specific strategies.

Serious side effects are very rare (0.1-1% of users). These include: acute pancreatitis (pancreas inflammation), cholecystitis (gallbladder inflammation/gallstones), certain thyroid conditions, severe dehydration. Regular medical monitoring (monthly check-ups) by Go Leaner catches these problems very quickly. Blood work checks pancreas substances (amylase, lipase) and liver function. ECG checks heart function.

Long-Term Safety

Research on patients using GLP-1 for 3+ years shows no unexpected side effects or problems. These medications are safe for long-term use. Go Leaner monitors you monthly: weight, side effects, blood work, blood pressure. If you want to become pregnant, you stop at least 2 months before trying to conceive. The medication is completely out of your system after 4-6 weeks.

Who Is Eligible for Weight Loss Injections?

Generally, adults with overweight or obesity are eligible for GLP-1 injections. BMI criteria are: BMI 30+ (obesity) is automatically eligible. BMI 27-29.9 (overweight) can be eligible if you have health risks such as type 2 diabetes, heart disease or family history of heart disease, sleep apnea, high blood pressure, high cholesterol, PCOS (polycystic ovary syndrome), or non-alcoholic fatty liver disease.

You are NOT eligible if you: are pregnant or planning pregnancy in the next 2 months, have had medullary thyroid cancer, have MEN syndrome type 2 (hereditary syndrome), have certain thyroid conditions, have severe kidney dysfunction, use certain medications. Go Leaner's doctors will discuss all this with you.

Medical screening is essential. At your first appointment, Go Leaner performs: complete medical history (previous diseases, medications, operations, family history), physical examination, blood work (glucose, cholesterol, lipids, liver function, kidney function), blood pressure measurement, ECG (electrocardiogram for heart function), possibly echocardiogram if you have cardiac risk factors. This ensures we know the medication is safe for you.

For pregnancy: if you want to become pregnant, you stop at least 2 months before you try to conceive. The medication is completely out of your system after approximately 4-6 weeks. This gives you sufficient time. After delivery, you can restart if you are not breastfeeding. Breastfeeding: safety for baby is not fully proven, so we advise against it.

Weight Loss Injections vs Other Methods

Diet

Diet alone: traditional diets (low-fat, low-calorie, keto) achieve an average of 5-10% weight loss over 6-12 months. Most people regain their weight because hunger becomes overwhelming. GLP-1 injections achieve 15-22% weight loss because they biologically suppress hunger, so diet adherence is much easier and more sustainable.

Exercise

Exercise alone: even intense exercise (5x per week, 60 minutes) typically achieves only 3-7% weight loss because your body preserves more weight than you think. Nutrition must change. GLP-1 injections make nutrition easier because hunger drops, so you can actually achieve a calorie deficit.

Behavioral Therapy

Behavioral therapy/psychology: cognitive behavioral therapy helps emotional eating but achieves an average of 8-12% weight loss. It helps eating patterns but cannot change biology (hunger). GLP-1 injections change hunger biology, so less appetite means less stress and emotional eating.

Surgery

Bariatric surgery (gastric bypass, gastric band, sleeve): achieves 30-35% weight loss permanently. However: it is invasive (surgery), serious risks (infection, bleeding, complications), permanent changes in digestion, potential deficiencies (vitamin B12, calcium, iron), expensive (EUR 15,000-30,000). GLP-1 injections achieve 15-22%, but are non-invasive, much safer, reversible, much cheaper.

Supplements

Supplements/natural products: most have no evidence. Glucomannan, garcinia cambogia, conjugated linoleic acid: research shows minimal effect (<2% weight loss). GLP-1 injections have massive evidence from clinical trials with thousands of patients.

Costs of Weight Loss Injections in Netherlands

Medication costs without insurance in Netherlands: semaglutide (Wegovy) EUR 400-500 per month, tirzepatide (Zepbound/Mounjaro) EUR 500-600 per month, liraglutide (Saxenda) EUR 300-400 per month. Annual costs for semaglutide: EUR 4,800-6,000. For tirzepatide: EUR 6,000-7,200. These are significant investments.

Insurance coverage in Netherlands: GLP-1 medications for WEIGHT LOSS are usually NOT covered by Dutch health insurers (VGZ, Hospital Group, Achmea, etc.). Ozempic may sometimes be covered for type 2 diabetes, but not for primary weight loss. This is an important limitation. This means you pay privately.

Go Leaner offers integrated treatment packages: medication + doctor consultations + monthly monitoring + nutrition coaching + psychological support. Packages start at EUR 199-249 per month and go up to EUR 499-599 per month depending on intensity. Medication costs are on top of this. This is more cost-effective than self-organizing.

Cost comparison: self-organizing costs: medication EUR 500 + pharmacy/billing hours EUR 50 + private doctor consultations EUR 100-150 per month + coaching EUR 0 (self-directed). Total: EUR 650-750/month. Go Leaner package: EUR 450-550/month all inclusive. Our integrated program is actually cheaper because everything is coordinated.

How Go Leaner Offers Injection Treatment

Step 1 - Intake and Screening (Week 0-1): Your first appointment is comprehensive. Go Leaner's doctor performs: complete medical history (previous diseases, medications, operations, family history), physical examination, blood work (glucose, cholesterol, liver function, kidney function, pancreas markers), blood pressure measurement, ECG for heart function. We determine which medication best fits your situation. This takes approximately 60-90 minutes.

Step 2 - Injection Training (Week 1-2): Once approval comes, you learn how to self-inject. A trained nurse: shows you the technique, answers questions, lets you practice with a dummy pen. You feel prepared and confident. If you are afraid of needles, we help you.

Step 3 - Dose Buildup (Months 1-5): Slow, gradual dose increase according to titration protocol. This minimizes side effects. Starts low, builds weekly to therapeutic dose over 4-5 months. Monthly check-ins with doctor: how are side effects, eating behavior, weight. Nutrition coaching starts: which foods help, which to avoid, how to eat small portions.

Step 4 - Maintenance and Optimization (Months 6+): You are now on full dose. Go Leaner's team focuses on optimization: weight loss progress, side effect management, nutrition coaching goes deeper, exercise planning, sleep hygiene, stress management. Monthly doctor consultations: weight, side effects, blood work checks.

Nutrition and Lifestyle Coaching: This is what makes Go Leaner different. It is not just about medication. Nutrition scientists help you build healthy eating habits—not crash diets, but sustainable changes. Exercise support (not heavy gym, but daily activity), sleep hygiene (poor sleep sabotages weight loss), stress management (stress leads to emotional eating), psychological support if you have emotional issues with eating.
