Ozempic price per dose (2026)

Ozempic comes in four doses: 0.25 mg, 0.5 mg, 1 mg and 2 mg per weekly injection. The manufacturer, Novo Nordisk, sets a recommended price that pharmacies can apply, but there is some variation in the actual price from pharmacy to pharmacy.

Guide prices (monthly amount at a Dutch pharmacy, 2026): 0.25 mg roughly 130 euros, 0.5 mg roughly 160 euros, 1 mg roughly 200 euros, 2 mg roughly 260 euros. Some online pharmacies are slightly lower, but often without supervision.

The difference per dose is because a higher dose contains more active ingredient. During the build-up phase (first 8 weeks) you therefore usually pay less than during the maintenance phase.

Coverage for type 2 diabetes

Ozempic is covered by basic health insurance when you have a formal diagnosis of type 2 diabetes and other oral blood-glucose-lowering medication (such as metformin and sulfonylurea derivatives) has insufficient effect. The treating doctor, usually a GP or internist, writes the prescription.

If covered, you only pay the deductible (eigen risico), which is 385 euros in 2026. After that, the medication costs are fully covered. You do have to meet the conditions again every year to keep your coverage.

Off-label use for weight loss: no coverage

Off-label means that a doctor prescribes a medicine for a different indication than the one it is officially registered for. In the Netherlands this is legally permitted provided it is properly justified. With Ozempic this happens frequently in patients with obesity who do not (or do not yet) meet the Wegovy reimbursement criteria.

With off-label use there is no coverage from your health insurer. The patient pays for the medication entirely out of pocket, plus (in supervised programmes) the cost of consultations and coaching — bundled at some clinics, but at others such as Go Leaner deliberately kept transparent and separate.

Cost difference between Ozempic and Wegovy

Ozempic and Wegovy contain the same active ingredient: semaglutide. The difference lies in the maximum dose (Ozempic up to 2 mg, Wegovy up to 2.4 mg per week) and the registration (Wegovy is specifically registered for weight loss, Ozempic for diabetes).

On a monthly basis, Wegovy at the highest dose is usually 50 to 100 euros more expensive than Ozempic at 2 mg. For those who do not receive coverage, Ozempic at 2 mg can be financially more attractive, although the official indication for weight loss is Wegovy.

Costs at Go Leaner

At Go Leaner you pay 199 euros per month for the complete supervised programme: medical intake consultation with a BIG-registered doctor, blood tests, monthly doctor check-ins and lifestyle coaching. You pay for the Ozempic itself separately through the pharmacy (guide price 130 to 260 euros per month, depending on the dose). That way you can see exactly what supervision costs and what medication costs.

The programme fee of 199 euros stays the same throughout your treatment; only your medication costs move with the dose (build-up phase cheaper than maintenance phase). The minimum programme duration is 6 months, for responsible medical monitoring.
