What is Rybelsus?
Rybelsus is the first and only oral GLP-1 receptor agonist available worldwide. It contains semaglutide, exactly the same active substance as Ozempic and Wegovy, but in tablet form. The tablets are manufactured by Novo Nordisk and are licensed for the treatment of type 2 diabetes.
Developing an oral GLP-1 was long considered technically impossible because the molecule is broken down in the stomach. Novo Nordisk solved this by combining semaglutide with SNAC (salcaprozate sodium), an absorption enhancer that gives the tablet a protected micro-environment in the stomach. This also explains why Rybelsus has to be taken so precisely on an empty stomach.
In the Netherlands, Rybelsus is licensed for type 2 diabetes, not for weight loss. Some doctors prescribe it off-label for obesity; in that case there is no reimbursement from basic health insurance and you pay for it yourself.
Dosing schedule
Rybelsus is built up in three steps to minimise side effects. Each dose is maintained for at least 30 days before you move up.
Month 1
3 mg
Starting dose. The aim is to get used to it, not weight loss.
Months 2-3
7 mg
Therapeutic dose. First effects on appetite and weight become visible.
Month 4+
14 mg
Maximum dose. Full effect on appetite suppression and blood sugar.
Important dosing protocol
- • Take on an empty stomach immediately after waking, before you eat, drink or take other medication
- • No more than 120 ml of water (half a mug)
- • Wait at least 30 minutes before you take or eat anything else
- • Do not crush or split — the tablet works through its structure
This protocol is stricter than for many other medications. If you do not follow it, absorption drops below 1% and the treatment does not work.
Rybelsus versus injections
The big question: do you choose a daily tablet or a weekly injection? It depends on your goals and preference.
| Rybelsus | Ozempic | Wegovy | |
|---|---|---|---|
| Form | Tablet, daily | Injection, once a week | Injection, once a week |
| Maximum dose | 14 mg | 2 mg | 2.4 mg |
| Average weight loss | 5-8% | 10-12% | 15-17% |
| Indication NL | Type 2 diabetes | Type 2 diabetes | Obesity |
| Dosing protocol | Strictly fasting | Flexible | Flexible |
| Cost/month | €110-170 | €106-190 | €140-190 |
| Reimbursement obesity | No (off-label) | No (off-label) | Sometimes (GVS criteria) |
Choose Rybelsus if: you cannot tolerate or do not want injections, your primary goal is diabetes control, or you want milder appetite suppression to make smaller changes.
Choose an injection if: maximum weight loss is the priority, you would find the strict Rybelsus dosing protocol hard to keep up, or if Wegovy is reimbursed in your case and is therefore cheaper.
Side effects and risks
Side effects are comparable to other GLP-1 medication and occur mainly during the titration phase. In most patients they disappear within 4 to 8 weeks.
Common (10-25%)
- Nausea
- Diarrhoea or constipation
- Abdominal pain, bloating
- Decreased appetite
Rare but serious
- Pancreatitis (inflammation of the pancreas)
- Gallstones or cholecystitis
- Worsening of retinal damage in diabetes
- Suspected MTC with a family history
Rybelsus tablets: cost, reimbursement and how to buy
Rybelsus is available in 3 mg, 7 mg and 14 mg tablets (30 per box). The pharmacy price per month varies by dose:
- 3 mg (starting phase, first month): approximately €110 — €130 per box of 30
- 7 mg (low maintenance): approximately €130 — €155 per box
- 14 mg (maximum maintenance): approximately €145 — €175 per box
Reimbursement for type 2 diabetes: Rybelsus is licensed on the Dutch market for type 2 diabetes and is included in the Medicines Reimbursement System (GVS). Basic health insurance reimburses it subject to conditions, usually once metformin proves insufficient.
Reimbursement for obesity without diabetes: off-label use (for weight loss only, without a type 2 diabetes diagnosis) is not reimbursed by basic health insurance. Some supplementary policies offer partial reimbursement of up to €300-€500 per year. Our BIG-registered doctor assesses whether reimbursement is possible.
How to buy Rybelsus? Only on prescription from a Dutch BIG-registered doctor. At Go Leaner you receive a personal prescription after a comprehensive intake (including blood tests and BMI measurement). You then collect it from a Dutch pharmacy or have it delivered discreetly to your home via our partner pharmacy.
Dosing protocol and experiences with Rybelsus
The effectiveness of Rybelsus depends entirely on the strict fasting protocol. Patients who do not stick to these rules often achieve only 30-50% of the potential effectiveness.
How to take Rybelsus correctly:
- Immediately after waking, on a completely empty stomach (no coffee, tea or water in the 30 minutes before)
- Swallow the tablet whole with no more than 120 ml of water (do not chew or break it)
- Wait at least 30 minutes before you eat, drink or take other medication
- Taking it at the same time each day helps build the habit and keeps absorption consistent
What patients typically experience in the first 3 months:
- Weeks 1-2: mild nausea in the mornings, reduced appetite already noticeable
- Weeks 3-4: most gastrointestinal complaints ease, weight loss starts (~1-2 kg)
- Months 2-3: stable pattern, appetite significantly suppressed, on average 3-5% weight loss at the 14 mg dose
- Month 6: most patients have lost 5-8% of their starting weight
When to switch to injections? If weight loss stalls after 3-6 months on Rybelsus 14 mg, or side effects become unmanageable, your treating doctor may advise switching to Ozempic, Wegovy or Mounjaro for a stronger effect.
