Stopping GLP-1: how do you prevent the yo-yo effect?

What happens in your body when you stop Ozempic, Wegovy or Mounjaro? A planned tapering protocol and the anti-yo-yo strategy that makes the difference between lasting results and weight regain.

In short

Stopping GLP-1 abruptly without a replacement strategy leads, for most people, to regaining 50-65% of the lost weight within 12 months. With a planned tapering protocol + coaching, that can be halved or reduced further. This article explains how.

What happens biologically when you stop?

GLP-1 medication such as semaglutide, tirzepatide and liraglutide works in three ways at once: appetite suppression via the brain, delayed gastric emptying (feeling full for longer), and favourable effects on insulin sensitivity. When you stop, all three effects disappear within 1-3 weeks, depending on the medication.

At the same time, your body has adapted to a lower weight during treatment. Hormones that drive hunger (ghrelin) are elevated, and hormones that signal satiety (leptin) are lower than normal. This is a built-in protective mechanism of the body against what it perceives as "famine". It is biology, not a willpower problem.

The net effect: after stopping, you are hungrier than before treatment, while your metabolism has become lower. Without targeted countermeasures, weight comes back quickly. That is the mechanism behind the yo-yo statistics.

The planned tapering protocol

At Go Leaner we work with a taper over 8-12 weeks combined with more intensive coaching. The broad steps:

Phase 1 — Preparation (weeks 1-2)

We evaluate whether you are ready to taper: target weight reached and stable for ≥3 months, eating habits and exercise pattern anchored, no underlying medical factor. Your coach works extra intensively on the anti-yo-yo anchors: protein intake (1.6-2.2 g/kg), strength training 2-3× per week, sleep quality and stress management.

Phase 2 — Stepwise dose reduction (weeks 3-8)

We taper the dose in steps: with semaglutide, for example, from 2.4 → 1.7 → 1.0 → 0.5 → 0.25 mg, one step every 2 weeks. With tirzepatide a comparable scale. The goal: let the body slowly get used to less medication without hunger suddenly surging.

Phase 3 — Stopping completely (weeks 9-10)

You stop completely. In the first 2-3 weeks after stopping, many people experience more hunger and some sluggish bowel adjustment. A short extension of coaching sessions helps you get through this period without falling back on old eating patterns.

Phase 4 — Maintenance and check-ins (from week 11)

We stay in touch — monthly for the first 3 months, then every 2-3 months. At the first signs of weight gain we intervene early: adjusting nutrition, exercise, possibly a temporary maintenance dose. Early action prevents a major relapse.

The 5 anti-yo-yo anchors

What makes the difference, in studies and in our practice, between people who maintain their weight and those who lose it, are five consistently maintained habits:

  1. Protein priority (1.6-2.2 g/kg): protects muscle mass, provides longer satiety, partly replaces the "full" signal that GLP-1 gave.
  2. Strength training 2-3× per week: builds or maintains muscle mass, increases resting metabolism, makes insulin response more favourable.
  3. Sleep 7-8 hours: lowers ghrelin and cortisol, improves satiety hormones. Statistically, poor sleep is the #1 hidden factor in weight regain.
  4. Structured meals: 3 meals + 1-2 snacks at fixed times prevents unnoticed calorie accumulation. On GLP-1 this was less necessary — now it is.
  5. Weekly weigh-in routine: not to weigh obsessively, but to spot an early increase within 2-3 kg and adjust — not waiting until 10 kg has been added.

When is restarting sensible?

Does ≥5% of your weight come back despite consistent effort? Do not wait until it has all returned. A conversation with your BIG-registered doctor about restarting at a lower dose, or a temporary maintenance dose, is a medically sound option — not a failure.

Stopping with support? We have a protocol

At Go Leaner we work from day 1 towards stopping without yo-yo. Coaching, nutrition and monitoring are part of the programme.
