Oral GLP-1 · in development
Orforglipron
The first non-peptide oral GLP-1 receptor agonist. A daily tablet with no fasting protocol and no injection — with phase 2 results comparable to Wegovy.
Important: as of May 2026, orforglipron is not EMA-approved and is not available from Dutch pharmacies. Eli Lilly expects FDA approval for obesity in 2026-2027. EMA approval typically follows 6-12 months later. For patients in the Netherlands, Wegovy or Mounjaro are currently the most effective EMA-approved alternatives.

What is orforglipron?
Orforglipron (development code LY3502970) is an experimental medicine from Eli Lilly — the same manufacturer as Mounjaro and the also not-yet-approved retatrutide. What makes orforglipron unique is that it is the first non-peptide GLP-1 receptor agonist.
Existing GLP-1 medications such as Ozempic, Wegovy and Saxenda are peptide medicines (small proteins) that are broken down in the stomach — hence the injection form. Rybelsus gets around this with an absorption enhancer, but requires a strict fasting protocol. Orforglipron is a small synthetic molecule that activates the GLP-1 receptor without being broken down — so you can simply take it as a tablet, with no timing restrictions and no injection.
Orforglipron is expected to make its mark on the GLP-1 medication landscape: it will be particularly attractive for patients who want to avoid injections without the strict fasting rules of Rybelsus, and who value the higher effectiveness (~15%).
Clinical evidence
The phase 2 GZGI study (Wharton et al., published in The New England Journal of Medicine, June 2023) showed weight loss of 9.4% to 14.7% after 36 weeks in 272 obese adults without diabetes, depending on the dose (12 mg to 45 mg daily). Placebo produced 2.3% weight loss over the same period.
Side effects were gastrointestinal and comparable to existing GLP-1s: nausea (32%), diarrhoea (20%), constipation (14%) and vomiting (10%). The discontinuation rate due to side effects was 10-17% at the highest dose.
Phase 3 ATTAIN-1 (obesity without diabetes) and ATTAIN-2 (obesity with diabetes) started in 2024-2025. Results are expected in late 2026. The ACHIEVE programme for type 2 diabetes is also ongoing.
Orforglipron versus Rybelsus and the injectable GLP-1s
How does this new non-peptide oral GLP-1 compare to existing options? The key figures side by side.
| Property | Orforglipron | Rybelsus (oral) | Wegovy (injection) |
|---|---|---|---|
| Active substance | Orforglipron (non-peptide) | Semaglutide (peptide) | Semaglutide 2.4 mg |
| Route of administration | Daily tablet | Daily tablet | Injection once a week |
| Fasting protocol | Not needed | Strict (30 min before breakfast) | N/A |
| Weight loss (phase 2/3) | 14.7% in 36 wks | 5-8% in 6 months | 14.9% in 68 wks |
| EMA approval | Expected 2027-2028 | Yes (2020, diabetes) | Yes (2022, obesity) |
| Available via Go Leaner | Not yet | Yes (off-label) | Yes, from €199/month |
| Best for | — (future) | Needle anxiety + diabetes | Obesity + maximum effectiveness |
When will orforglipron be available to buy in the Netherlands?
Timeline (based on Eli Lilly investor communications):
- 2025 — phase 3 ATTAIN-1 and ATTAIN-2 trials started
- Late 2026 — expected readout of topline results
- Early 2027 — FDA application for the obesity indication (US market)
- 2027-2028 — EMA application for Europe
- Expected 2028 — market authorisation in the Netherlands via pharmacies
Until then, the legal, EMA-approved alternatives in the Netherlands are: Rybelsus (oral semaglutide, daily tablet), or for maximum effectiveness an injectable GLP-1 such as Wegovy or Mounjaro.
Watch out for: providers already selling orforglipron through "research peptides" shops or grey-market channels. Not only is this illegal under the Dutch Medicines Act (Geneesmiddelenwet), but unknown doses and impurities also make it medically unsafe.
Want to start an oral GLP-1 now?
For patients who do not want injections, Rybelsus is already EMA-approved and available in the Netherlands. Our BIG-registered doctors can prescribe it where indicated.
